Email Header - "unknown"

On inbound messages, the following header is inserted. I've replace the domain, IP address and email address:

 

Received: from domain.com (unknown [ip-address])

by Websense Email Security Gateway with ESMTPS id 697B94A8BBC

for <user@domain.com>; Thu, 14 Jun 2012 11:03:02 -0700 (PDT)

 

Email is routing properly.

Why would "unknown" be inserted before the IP address? 

We are using 7.6.2.

1.We can see that format very often, like:

Received: from pps.filterd (PFPTTest [127.0.0.1])

'PFPTTest' is the hostname of the sender, so 'unknown' shoud not be inserted.

2. What's the sender's IP? We can test it to verify if it is caused by the sender's mail server or not.

The message was received by Websense from our Ironport appliance (our primary email filter).
